成大斗六分院整形外科張子彥醫師表示，乳癌有年輕化的趨勢，但乳癌的治療效果佳，使乳癌病人成為「較年輕且存活也較久」的族群，這也凸顯除切除乳房以求存活率外，能夠重建乳房以恢復乳癌患者的美麗跟信心變得非常重要。

乳癌為我國婦女癌症發生率第1位，死亡率占到第4位，每年約有1萬多名新診斷乳癌患者，也有約2千名死於乳癌。但張子彥醫師指出，乳癌發生率雖高，但因為篩檢的普及，與抗荷爾蒙、標靶藥物的進步，早期乳癌接受治療的存活都比起其他癌症來得好，第一、二期乳癌切除後的5年存活率接近9成。

針對乳癌的治療，成大斗六分院乳癌團隊有豐富的經驗。張子彥指出，團隊以內視鏡做乳癌微創切除，並配合做乳房重建手術，將手術切口隱藏在腋下及乳暈周邊，內視鏡由腋下切口導入來輔助乳暈的切口做乳房切除，將胸大肌及前鋸肌翻起，把義乳保護於其下而完成重建。

張子彥表示，在術中會完成前哨淋巴結的冷凍切片確認是否有轉移，若有則先放置組織擴張器，等電療完成後再置換為義乳；同時也會做乳頭底部的冷凍切片以確定是否有癌細胞侵犯，約有1成病人會因為癌侵犯而無法保留乳頭。

通常乳房全切除除了切除乳腺組織外，還會包括一大片皮膚，及乳頭、乳暈；乳房重建得額外轉移背部或腹部的皮膚及脂肪，義乳的使用須搭配健康側乳房的大小，最後再重塑乳頭形狀並刺青來得到新的乳頭。但轉移使用的皮膚及重建的乳頭，永遠比不上原有的自然。

張子彥醫師說，會盡量保留原本的乳頭及皮膚，讓乳房重建更為自然。而目前研究也都指出，在大多情況下保留皮膚跟乳頭是安全的，並不會增加癌症轉移或復發的機率。

張子彥表示，若病人腹部較豐腴或有剖腹產疤痕的病人，適合使用腹部皮瓣轉移，不僅可以避免因義乳排斥反應所產生的變形或不舒服，更可以同時雕塑腰部曲線，得到如同肚子拉皮的效果，一舉兩得。

張子彥醫師提醒指出，義乳重建的病人術後應配合簡易的按摩，並做磁振造影的追蹤；若是使用腹部皮瓣重建的病人，則是配合超音波的追蹤，並同時針對癌細胞的特性持續使用抗荷爾蒙、標靶藥物、化學治療等。
